Privacy Policy
Formiva is a privacy-first forms and workflow platform operated as a beta at formiva.suherman.net. This policy describes how the beta handles information.
What we collect
- Form definitions (FormSpec) and settings that operators create.
- Responses submitted through public form links. Operators choose the questions; answers may include personal information if asked.
- Technical logs needed to operate the beta (timestamps, request paths, error diagnostics). We do not sell personal data.
Public respondents
Public forms do not require an account. Optional soft fingerprints for abuse control are never shown as identity in results.
Where data lives
This beta runs on Google Cloud (Australia). Form and response data for the hosted edition is stored in the Formiva Cloud Run data plane backing formiva-api.suherman.net.
Retention and deletion
Beta data may be reset without notice. Operators can delete forms (which removes associated responses) through the operator apps when available. Contact the operator below to request deletion of beta data.
Children
The beta is not directed at children under 13. Do not knowingly submit children’s personal information.
