User guides · Step 2 of 4
Create and publish a form
You must be signed in. Open Your forms.
1. Create the form
- Tap New form.
- Enter a title — for example Office lunch preferences.
- Tap Create. Formiva opens the form detail page in DRAFT.
2. Add details
- Optionally edit the description.
- Turn on Show public results if respondents may see live totals after they submit.
- Tap Save details.
- Copy the Public link (it looks like
/f/your-title-abc123) — it works after publish.
3. Build fields
- Stay on the Builder tab.
- Choose a field type under Add field: Text, Text area, Single choice, Multiple choice, Yes / No, or Rating.
- Select a field to edit its label, required flag, options (for choice), or rating scale.
- Use Up / Down to reorder, or Delete to remove.
- Tap Save fields. Structural edits are locked after publish until you close the form.
4. Publish
- Confirm you have at least one field saved.
- Tap Publish & collect. Status becomes COLLECTING.
- Share the public link with respondents (email, chat, QR, etc.).
- When finished, tap Close form so new answers stop.
Example from our walkthrough
We created a draft with four fields:
- Your name (text, required)
- Preferred lunch (single choice: Sandwich / Salad / Soup)
- How hungry are you? (rating 1–5)
- Any dietary notes? (text area)
After publish, status was COLLECTING and the public URL was ready to share.
