Fill in a published form

Respondents do not need an account. They only need the public link.

1. Open the link the operator shared — for example https://formiva.suherman.net/f/office-lunch-preferences-7x50ea.
2. Read the title and description.
3. Answer each question. Required fields must be completed before submit.
4. For choice questions, pick one option (or several if it is multiple choice). For ratings, choose a number on the scale.
5. Tap Submit (or the form's custom submit label).
6. You should see a thank-you message. If public results are enabled, you may also see live totals.

If the form is closed

After an operator closes the form, the public page explains that collection has ended. New answers are rejected.
